You'll have experience being a Registered Nurse who can manage prospective clients, close new clients, create care plans, sign up new clients, and train caregivers for their first shifts.
You'll want to follow up and manage care to ensure we keep our clients out of the hospital and aging at home.

Key Responsibilities Conducts the assessment and reassessment of patients, including updating of care plans and interpreting patient needs, while adhering to Company and/or AHCA license regulations Provide admission, care management, and follow-up nursing visits for clients.
Administer on-going care and care management for each patient every 60 days and provide necessary follow-up as directed by the Director of Operations Coordinate/oversee/supervise the work of Home Health Aides, Certified Home Health Aides and Personal Care Workers and provides written personal care instructions/care plan that reflects current plan of care.
Monitor the appropriate completion of documentation by home health aides/personal care workers as part of the supervisory/leadership responsibility.
Receives Medicaid referrals.
Reviews available patient information related to the case, including disciplines required, to determine care needs.
Accountable to ensure patients meet the admission criteria and makes the decision to admit patients to service Report patient care/condition/progress to the patient's responsible persons and team on a continuous basis.
Prepare appropriate medical documentation on all patients, including any case conferences, patient contacts, hospitalizations, changes in conditions updates, and care plan changes.
Prepare visit/shift reports, updates/summarizes patient records and confers with other health care disciplines in providing optimum patient care.
Assists clients in establishing immediate and long-term goals, in setting priorities, and in developing patient Plan of Care (POC).
Monitors cases to ensure documentation is in compliance with regulatory agencies and requirements of the third party payers.
Coordinates communication between team members/Care Managers/caregivers to ensure the appropriateness of care and outcome planning.
Handles necessary employee corrective action and discipline issues fairly and objectively, in consultation with the Human Resources Department and the Executive Director/Director of Operations.
Coaches staff and recommends in-service education programs, when needed.
Ensures adherence to internal policies and standards.
Assesses staff education needs based on own review of clinical documentation in addition to feedback and recommendations by Utilization Review staff.
Upon completion of the assessment, creates and conducts regular staff education as needed.
Coordinate and recommend appropriate care, encompassing various healthcare personnel (such as Physical Therapists, Occupational Therapists, Home Health Aides, and external providers).
Implement patient care plan in conjunction with patient and family to assist them in achieving optimal resolution of needs/problems.
Manage and Train our Home Health Aides through the AHCA home health aide competency course and program Job Type:
