Summary:
The Discharge Planner I provides assessment, linkage, monitoring, planning and advocacy for adult and or children experiencing acute mental health and or substance abuse issues in an acute care inpatient setting.
Possesses a clear understanding of discharge and transition planning and linkage with community resources.
Oversees legal status issues related to the individuals involuntary or in accordance with the Baker Act regulations.
Provides support as active member of the treatment team.
Travel may be required.
Will perform other duties as assigned by supervisor.
Required experience includes 1 year of Behavioral Health and 2 years of Child and Adolescent (SIPP Only).
